Phase 4: Evaluation/Reflection
It is crucial to document the intermediate and long-term outcomes of the initiative. An evaluation should provide feedback to decision makers and participants about needed program modifications and improvements.
In other words, an evaluation should not be an afterthought. It should be integral to the program planning and implementation processes from the outset. A well-designed evaluation should, at a minimum, provide for measuring progress at appropriate intervals, interpreting data accurately and reporting results to stakeholders in formats that are easy to understand.
